Jacob Levi Howard, 16, of Alicia, Arkansas, passed away December 22, 2015 at St. Bernard's Regional Medical Center. He was born July 27, 1999 in Hoxie, Arkansas to James and Jamie Howard.
Jacob was preceded in death by his grandmother, Juanita Roberts; great grandparent, Vaughn Gammil.
Survivors include his parents; paternal grandparents, Harold and Kay Howard, Pat Howard; great grandmother, Ruby Gammil; brother, Tony Mashburn; sisters, Jennifer Hewitt, Michelle Gruhler, Jessica Howard.
The family will receive friends from 5:00 – 7:00 p.m. on Sunday, December 27, 2015 at Phillips Funeral Home. Funeral services will be at 10:00 a.m. on December 28 in the Center Hill Chapel of Phillips Funeral Home with David Stallings officiating. Burial will follow in Markum Cemetery.
